Your counselor or teacher reference letters and form need to be mailed to Vicki Dilley.  Be sure and choose someone who knows you fairly well.  If your school counselor does not know you well, ask a teacher to write a report for you.  Include with that a stamped envelope addressed to Vicki Dilley, 714 Highland Avenue, Northfield, MN 55057.  You only need one copy of these.  Be sure and have them sign in blue ink.

Please also fill out the Supplemental Health Letter along with the application.

Applications will be due October 10, 2011.  Deliver completed applications to Vicki Dilley at the address above.
